The Hospital Value-Based Purchasing (HVBP) program
The HVBP program rewards hospitals with incentive payments for the quality of care they give to Medicare patients. The HVBP program was enacted through the Affordable Care Act in 2010 and began in 2012.
Domains and measures
In 2018, there are approximately 20 measures in the HVBP program that span four different domains of hospital care quality. The list is ever expanding and will include approximately 25 measures by 2023. Let's take a look at each domain and measure here.
The clinical care domain
The clinical care domain measures clinical care quality predominantly by using mortality measures. The mortality rate refers to the patient death rate for a specific disease. There are five mortality measures used in this domain (listed here). The sixth measure is the complications rate for the total hip/knee arthroplasty (a.k.a. replacement) procedure:
